It is important that we take
joint responsibility for achieving whatever changes you want to make.
I will therefore discuss all aspects of your treatment with you, right from the
time you make your first appointment so that you can benefit from being fully
involved in your change process .
In order to facilitate this mutually-beneficial arrangement,
I will give you a written contract similar to the one below, right at the
beginning of your first session, which provides
a professional structure to the therapeutic relationship:

RULES:

I reserve the right to record all or part of
each session.

I may use some of the information in
consultation with my supervisor however I will not identify you by name.

Sessions are confidential, however, I reserve
the right to break confidentiality if: a) I feel you have given me information
which seems to me to indicate threat to the health or life of yourself or
anybody else. If this happens, I may report that information to the relevant
authorities e.g. your doctor, the police or emergency medical services or b) If
a judge requires me to answer questions about you in a court case.

You agree not to commit physical harm against
yourself, me or anyone else on the premises and you will not cause damage to the
building, fittings or furniture.

You agree to attend the sessions free from the
influence of alcohol or non-prescription drugs.

In accordance with both the Institute of
Transactional Analysis and National Council for Psychotherapists' Code of
Ethics, any sexual relationship between counsellor and client is prohibited.

You agree to pay the agreed fee in full at the
end of each session.

If you miss a previously arranged session for
any reason, I reserve the right to charge all or part of my fee for the time
concerned.
